Dr. Floyd was born in Gooding, Idaho and raised in Southern Idaho and Oregon. Dr. Floyd attended the University of Idaho and went on to earn his Doctorate in Podiatry at the Ohio College of Podiatric Medicine. Dr. Floyd began practicing in Lynnwood, WA in 1984 where he has worked at the Alderwood Ankle and Foot Clinic for the past 36 years. In 2002 Dr. Floyd expanded his practice to Lake Stevens, at the Lake Stevens Ankle and Foot Clinic. This enabled his patients that lived in the North end to access their Foot and Ankle care with less commute.
Dr. Floyd is a Board Certified Podiatrist and Foot and Ankle Surgeon and is a Diplomate of the American Board of Podiatric Surgery, and Fellow of the American College of Foot and Ankle Surgeons. In 2002 Dr. Floyd completed a post graduate Course in Medical and Surgical Techniques of the Foot, Ankle and Leg at Northledge Regional Medical Center, Tucker, GA. Dr.Floyd is affiliated with the Swedish Hospital in Edmonds, WA and is on their Surgery Committee.
Dr. Floyd has been involved with Athletics his entire life and also specializes in Sports Injuries. He enjoys treating athletes of all Sport types. Dr. Floyd is also a highly trained Gold Medal Squared Select Volleyball Coach for 12 years. He has treated many premier volleyball athletes around the region. His philosophy is keeping the athlete going and preventing future injuries. Dr. Floyd is also a Private Pilot and enjoys flying, water skiiing and kyaking with his family and his Chocolate labs.
Sandi C. Nagata, D.P.M., A.B.P.M.

Dr. Nagata was born in Kirkland, WA and raised in Issaquah, WA. She attended the University of Southern California (USC) where she earned her Bachelor of Science in Environmental Studies with an emphasis in Biology. Dr. Nagata achieved her doctorate degree at Temple University School of Podiatric Medicine in Philadelphia, PA. She stayed in the Philadelphia area to complete a 3-year Podiatric Medicine and Surgery Residency at Crozer-Keystone Health System. Dr. Nagata then continued on to do a prestigious orthopedic fellowship at Sinai Hospital in Baltimore, MD in the Rubin Institute for Advanced Orthopedics. Here she trained alongside the Orthopedic Fellows in limb deformity correction and served as her class' Chief Fellow. During her time here she also went with a team on an orthopedic medical mission to Santo Domingo, Dominican Republic at CURE Hospital.
Dr. Nagata has been behind the scenes of athletics for many years after discovering she was better at treating athletes than being one while attempting to play basketball in high school. She continued student athletic training at USC, primarily with football and women's tennis. Her love of sports medicine grew with additional experience while in Philadelphia, helping to provide podiatric care at the collegiate and professional level. Exposure to helping keep athletes on the field or court has translated to her passion for wanting to keep all patients healthy when it comes to taking care of their feet.
Dr. Nagata has been practicing alongside Dr. Floyd at the Alderwood Ankle and Foot Clinic since 2013. Dr. Nagata is a Board Certified with the American Podiatric Medical Association. She is a member of the American Podiatric Medical Association and the Washington Podiatric Medical Association.Dr.Nagata is affiliated with the Swedish Hospital in Edmonds, WA and is on their Emergency Medicine Committee. ON her time off she can be found playing outside with her daughter, running, paddle boarding or taking a vacation in Hawaii visiting extended family.
Angela J.Zhang, D.P.M.

Dr. Angela Zhang was born in Shanghai, China and raised in New York City. She attended Pennsylvania State University in University Park and graduated with a Bachelor of Science degree, with dual majors in Biology and Psychology and a minor in Business. She received her Doctorate of Podiatric Medicine degree from Temple University School of Podiatric Medicine in Philadelphia, PA. She did a three-year forefoot, rearfoot, and ankle surgical residency at Crozer Keystone Health System in Philadelphia, PA.
Her residency training focused on diabetic limb salvage, sports medicine, traumatic injuries, and foot and ankle reconstructions. She also received training at the Rubin Institute of Advanced Orthopedics in Baltimore, Maryland on various pediatric deformities, minimally invasive bunionectomies, and advanced limb reconstruction. She has also spent time working with the official team podiatrist of the Philadelphia Eagles and podiatry consultant of the Philadelphia 76ers.
Dr. Zhang is board qualified in foot and ankle surgery by the American Board of Foot and Ankle Surgery. She is a member of the American College of Foot and Ankle Surgeons, the American Podiatric Medical Association, and the Washington Podiatric Medical Association. She currently resides in Bothell, WA. In her free time, you may find her volunteering at sports events or healthcare fairs, spending time with her family, hiking, reading, and watching foreign films. She is dedicated to helping patients get back on their feet and return to pain-free activities. She is excited to join the amazing team of doctors at the Lake Stevens Ankle and Foot Clinics.
Michael H. Ham, D.P.M.

Dr. Michael Ham was born and raised in San Jose, California. He attended the University of California Davis where he earned his Bachelor of Science in Neurobiology and Physiology. Upon completing his undergraduate studies, Dr. Ham moved to Philadelphia, PA to obtain his doctorate degree at Temple University. Afterwards he remained in Philadelphia to continue his training at Thomas Jefferson, where he completed a comprehensive three-year Podiatric Medicine and Surgery residency.
Dr. Ham's experience as an athlete at the high school and collegiate level has made him keenly aware of the difficulties foot and ankle issues can impose on one’s quality of life. He believes in individualizing treatment plans to fully incorporate patient's goals and expectations on the way to a pain free lifestyle.
Dr. Ham is board qualified with the American Board of Foot and Ankle Surgery in both foot and reconstructive rearfoot and ankle surgery. He is a member of the American College of Foot and Ankle Surgeons, the American Podiatric Medical Association, and the Washington Podiatric Medical Association. Dr. Ham is eager to return to the west coast to join the Alderwood Ankle and Foot Clinic. During his free time Dr. Ham enjoys exercising, mountain biking, and exploring new restaurants.
